IMPHAL, Sep 16: Shija Hospitals & Research Institute successfully managed a rare and high-risk case of Complete Heart Block (CHB) in a pregnant woman, ensuring the safe delivery of a healthy baby and the well-being of the mother through a coordinated multidisciplinary approach.
Complete Heart Block (CHB) is a serious condition in which the heart's natural electrical signals do not pass normally, causing the upper and lower chambers of the heart to beat out of sync. As a result, the heart can slow down dangerously and even stop for a short period. CHB may be congenital, present at birth, or acquired later in life. Congenital cases are very rare, said the Shija Hospitals.
The pregnant woman, who is a nurse, was diagnosed with congenital CHB years earlier during a routine medical check but remained without symptoms, so a pacemaker was not required, said the hospital.
Pregnancy, however, had increased the risk. A sudden slowing of the heart, irregular heartbeats, or even the heart stopping could have endangered both mother and baby, said the hospital.
Although there was no obstetric need for a Caesarean section, the patient opted for it to minimize risks.
A team including Dr Nandeibam Yohen, Consultant in Reproductive Medicine, Dr Kennedy Taiyenjam, Dr Kajal Momin, and Dr Yashobanta, Consultant Anesthesiologists and Critical Care and Dr Sumidra Laishram, Consultant Cardiologist carried out the surgery.
The anaesthesia team employed epidural anaesthesia, which offers safe control of blood pressure and heart rate compared to spinal or general anaesthesia. Invasive arterial blood pressure was monitored both intra-operatively and postoperatively, and a temporary pacemaker was kept on standby as a precautionary measure. Both mother and baby remained stable during surgery and recovery, said the hospital.
